Fertility Laboratories of Colorado (FLC)

Fertility Laboratories of Colorado (FLC) consists of an in vitro fertilization laboratory and a clinical laboratory, which is accredited by the Commission on Laboratories Accreditation (COLA). In addition to providing In Vitro Fertilization procedures for patients at the Colorado Center for Reproductive Medicine, FLC provides clinical laboratory services for our patients as well as patients that have been referred to us from referring physicians or providers.

For patients referred from a physician outside CCRM the actual services received will be based the referring physician’s treatment plan. Services are provided by appointment only and require a prescription from the referring provider.

For patients: Referral Checklist

1. Choose a location and call the office to schedule an appointment. For after hours scheduling call 303-788-8300. The answering service will forward the message to FLC.
2. Obtain a physician order for the tests/procedures and bring them to the appointment, or the physician can fax the order to the appropriate location listed above.
3.  Bring a photo ID for all andrology testing or procedures.
4. Bring a copy of your insurance card, your photo ID, and a copy of a referral (if required). If we participate in an insurance plan we will bill the insurance company directly.

Billing/Payment for Services

• We will bill insurance directly if FLC participates in the patient’s plan, and the plan covers the services provided. Co-pays or co-insurance will be collected at the time of service.
• If FLC does not participate the patient’s insurance plan, the individual will need to pay for the services at the time they are provided. FLC will provide the patient with claim forms and the patient can submit this directly to the insurance company for reimbursement.
• If the patient does not have insurance benefits for infertility services, services will need to be paid for at the time they are provided. We accept cash, personal checks, MasterCard, Visa and Discover credit cards.
